VAT refund for foreign patients: when it applies and when it does not

Korea refunds VAT on some cosmetic and plastic surgery services received by foreign patients. It comes to close to 10% of what you pay, so it is not a rounding error in your total — but people who do not know the conditions simply fly home without it.

Three things all have to line up

• You — a foreign national not residing in Korea. Length of stay and status can put you outside the scheme.
• The clinic — not any clinic, but one designated and registered for the refund. This is the condition people fail most often.
• The treatment — an item on the eligible list. Even among cosmetic procedures, the scope is defined.

So, one sentence to ask before you book

"Is this clinic registered for the foreign-patient VAT refund, and is the treatment I am booking an eligible item?"

Ask both halves. An eligible clinic with an ineligible treatment refunds nothing, and so does the reverse. Getting the answer before you pay changes your total materially.

There are two ways it actually reaches you

• Deducted at the clinic — the tax comes off at payment. Simplest.
• Refunded on departure — you carry the clinic's refund paperwork through the process at the airport. Without the paperwork there is no claim.

Which one applies varies by clinic, so settle it before paying. If it is the departure route, keep the documents in your hand luggage, not your checked bag.

Where it usually goes wrong

• Hearing that the refund "is handled" and leaving without the documents.
• Passport details missing from the receipt, so nothing matches later.
• Paying as a package, so eligible and ineligible items sit inside one amount. Paying item by item avoids this.
• Finding out at the gate. Start the airport process with time in hand.
