Ulthera or Thermage: sagging and slackening are two different problems

These two come up together in almost every consultation, and searching returns a flood of "which is better" articles. But they are not competitors. They reach different layers, so they solve different problems. The question is not which is better but which problem you actually have.
They work at different depths
Focused ultrasound concentrates energy at a point and heats a deep layer — down to the fibrous sheet that supports the face. As that layer contracts, it lifts what has descended.
Radiofrequency heats a broader area of the dermis. As collagen responds, the effect is closer to the skin itself tightening and thickening. It is used less to raise the jawline than to change the quality of skin that has thinned and slackened.
Which one is your problem?
- What bothers you in the mirror is a blurred jawline and cheeks that have dropped → the lifting side
- What bothers you is not descent but thin, slack skin with more fine lines → the tightening side
- Both → agree the order and the interval at the consultation. Doing everything on one day is not automatically better.
The pain and the recovery differ in kind
Because ultrasound concentrates energy at a point, it produces brief deep aches that pass. Radiofrequency heats broadly, so it is a repeated hot, stinging sensation. Which hurts more varies by person and depends heavily on the number of shots and the intensity setting.
Downtime is short for both, but it is not zero. Use the recovery figures on the cards above and avoid stacking either right before something that matters. Swelling can last a day or two, and bruising happens occasionally.
The price gap is mostly about shots
Both use consumables — a cartridge for ultrasound, a tip for radiofrequency — and how many shots are delivered accounts for most of the price. Under the same treatment name, 300 shots and 600 shots are different treatments at different prices. Comparing quotes by treatment name alone tells you nothing.
What a quote must specify
- Shot count — how many, and how they are divided between areas.
- Consumable opened in front of you — can you watch the cartridge or tip being opened? These have a fixed number of uses.
- Who performs it — with these two, where and how the energy is placed changes the result substantially.
- Which device — you can look the device up by product name in Korea's licensing database.
Be wary of "one session does it all"
Lifting treatments contract tissue with heat, and the change completes over the following months as collagen rebuilds. The day of treatment is not the final result. Conversely, where descent is already advanced, there is a limit to what non-surgical lifting can reverse. Aligning expectations at the consultation matters more here than almost anywhere else.
